WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?

The only insurance company that we are In-Network with is Medicare. Otherwise, we have chosen to be an Out-Of-Network provider as we found the limitations from insurance to be restrictive to the quality of care we desire to provide. If you would like us to check on your out-of-network benefits, we will gladly do this for you and can still submit to your insurance if that is your preference. Please feel free to contact us to ask about our self-pay and package rates if you will not be using insurance.

WHAT ARE THE BENEFITS OF BEING OUT OF NETWORK WITH INSURANCE?

Being out-of-network with insurance allows us to provide the highest quality of one-on-one care for our patients. Each and every session will be spent with our Doctor of Physical Therapy who will guide you in your individualized rehab program to make sure you can return to whatever it is you enjoy doing most!

DO I NEED A DOCTOR’S REFERRAL?

You do not, Colorado is one of 20 states that allow patients unrestricted access to a physical therapist. No type of physician referral is required for a physical therapist to treat a patient. If, after evaluation, it is determined that further medical evaluation is required, we will reach out to your doctor on your behalf to discuss our findings.

DO I NEED TO DO A NEW PATIENT EXAMINATION?

If it is your first time seeing Dr. Schroeder for physical therapy services, then yes, a new patient examination is required. This allows us to collect the necessary patient information for our system and allows Dr. Schroeder to complete a thorough examination and develop a comprehensive treatment plan. If you have been to our clinic before and have a new injury, we recommended booking a 60 minute follow-up so that a re-evaluation can be performed of the new injury. If it is your first time to the clinic, our intake forms are provided below for your convenience to complete them at your leisure prior to your session.
